QUALIFICATIONS

SHORT DESCRIPTION
The Area Director of Admissions and Marketing will be responsible for driving census growth and managing admissions for multiple Skilled Nursing Facilities through community outreach and relationship building with hospitals and referral sources.
EmpRes Operated by Evergreen is looking for an Area Director of Admissions and Marketing responsible for supporting and executing the admissions and marketing strategies across multiple Skilled Nursing Facilities (SNFs). This role involves knowledge and understanding of the full admissions cycle, building and nurturing relationships with referral sources, and driving census growth through community outreach and hospital partnerships. The ideal candidate will be proactive in hospital bedside visits, market ownership, and strategic collaborations that enhance patient flow and facility visibility.
ESSENTIAL FUNCTIONS
- Manage and oversee the referral conversion process across assigned SNFs, ensuring timely follow-up and smooth transitions.
- Conduct hospital bedside visits to evaluate current and potential patients for admission into the SNF.
- Build and maintain strong relationships with hospitals, physicians, discharge planners, social workers, and case managers to facilitate new referrals.
- Lead community outreach efforts, participating in Chamber of Commerce meetings, Rotary meetings, retirement connections, and other local organizations to strengthen market presence.
- Attend hospital post-acute collaborative meetings to represent the organization and identify opportunities for partnership and clinical programming.
- Identify partnership opportunities with hospitals for clinical programming and niche marketing to increase referrals.
- Communicate daily with facility admissions personnel, Executive Directors, and the Regional Director of Marketing on census status and referral updates.
- Identify barriers that hinder census growth and work proactively to resolve them.
- Facilitate referral conversions into admissions by working closely with discharge planners and case managers to ensure all paperwork is completed and patients are transitioned smoothly.
- Participate in daily stand-up meetings at assigned facilities at least once a week to stay informed of facility operations and collaborate with the team.
- Enter referrals into the PCC CRM system and ensure accurate and timely follow-up.
- Fill-in for admission coverage at designated facilities when Admission personnel is on PTO/Sick or absent.
OTHER DUTIES
- Other duties as assigned and appropriate to the position.
KNOWLEDGE, SKILLS, AND ABILITIES
- Full knowledge of the SNF admission process, including reviewing referrals, entering leads into PCC CRM, and managing admission paperwork.
- Proven experience in building and maintaining relationships with hospital referral sources and driving census growth.
- Must understand medical terminology and clinical capability grids (Green/Yellow/Red)
- Ability to multi-task in a fast-paced environment, prioritizing tasks based on urgency.
- Strong organizational, leadership, and communication skills.
- Ability to travel frequently between facilities and hospitals.
- Experience in community outreach and business development, including participation in local events and organizations.
- Strong computer skills including the use of Teams, Outlook, Word, Excel.
EDUCATION, LICENSURE, AND EXPERIENCE
- Current Driver’s License
- Minimum of 3-5 years of experience in admissions, marketing, or a related field within the Skilled Nursing Facility (SNF) industry.

- General office environment. Flexible working hours. Position involves sitting, standing, lifting, bending, walking, driving, and moving frequently during working hours. Position may require overtime or evening/weekend hours.
