QUALIFICATIONS

Your work will be instrumental in promoting our services and ensuring seamless transitions for prospective residents.
Your Impact as a Clinical Liaison
In this role, you will:
- Develop and Strengthen Referral Partnerships: Dedicate time each week to connecting with hospital discharge planners, case managers, physicians, and social service agencies to build strong referral networks.
- Provide Clinical Expertise: Educate referral sources and community partners about our long-term care services, ensuring they have a clear understanding of the care we provide.
- Conduct Resident Evaluations: Assess potential residents for appropriate placement and services, working closely with the admissions and clinical teams to ensure smooth transitions.
- Build Community Awareness: Deliver presentations on long-term care, aging, and other healthcare topics to local community groups.
- Facilitate Admissions: Conduct facility tours and guide potential residents and families through the admissions process, ensuring they feel supported every step of the way.
- Maintain Accurate Records: Document interactions with referral sources and maintain up-to-date records of admissions progress and outcomes.
- Provide Reporting: Prepare weekly reports to track marketing efforts, admissions progress, and share updates with leadership.
- Promote Community Relations: Support internal initiatives to strengthen relationships and enhance community awareness.
What Makes You a Great Fit
We’re seeking someone who:
- Holds an active RN, LVN, or healthcare-related certification (preferred but not required).
- Has experience in healthcare marketing, admissions, or community relations, with a strong understanding of the long-term care industry.
- Possesses excellent communication skills, with the ability to engage effectively through written, verbal, and public speaking channels.
- Demonstrates clinical knowledge and the ability to assess potential residents for appropriate services.
- Is self-motivated, organized, and capable of working independently to meet goals and deadlines.
- Can commit to frequent local travel to build and maintain community connections and attend events.
