Position Summary


The Community Care Outreach Coordinator is a key member of the Caremosphere team, dedicated to expanding our care network through strategic outreach and relationship building. This role is essential for establishing and nurturing partnerships with healthcare professionals, including care managers and physicians, and for generating new client referrals to enhance community health outcomes.


Position Qualifications


  • Prior experience in community outreach, healthcare, marketing, or a related field is highly valued.
  • Strong interpersonal and networking skills to effectively build and maintain relationships.
  • Proficiency in using MS Office and CRM software to manage relationships and track engagement results.
  • Ability to travel locally to meet with potential partners and participate in community events.
  • Background check clearance as required for roles involving direct community interaction.

Key Responsibilities


  • Partner Engagement
    • Develop and maintain relationships with healthcare providers, community leaders, and other referral sources.
    • Regularly attend professional gatherings and community events to promote Caremosphere’s services and initiatives.


  • Referral Development
    • Implement targeted strategies to increase client referrals, including presentations and informational sessions about Caremosphere’s services.
    • Maintain a detailed database of contacts and interactions to streamline follow-up activities and maximize referral potential.


  • Community Programs
    • Coordinate and lead community outreach programs that highlight Caremosphere’s commitment to accessible and high-quality care.
    • Collaborate with local organizations to sponsor events and workshops that align with community health needs.


  • Outreach Coordination
    • Plan and execute outreach campaigns in collaboration with the marketing team to ensure consistent messaging and branding.
    • Analyze the effectiveness of outreach efforts and adjust strategies based on community feedback and engagement metrics.


  • Reporting and Documentation
    • Provide regular updates to Caremosphere leadership about outreach activities, partnership developments, and new client engagements.
    • Document all outreach activities in compliance with Caremosphere’s operational standards and privacy guidelines.


  • Development and Training
    • Participate in ongoing training and professional development opportunities to stay informed about healthcare trends and outreach techniques.
    • Contribute to training materials and sessions for new staff members joining the outreach or community care teams.
