Job Summary

The Community Liaison is crucial in building and maintaining positive relationships between our Skilled Nursing Facility (SNF) and the surrounding community. This position serves as a representative of the SNF and is responsible for establishing connections with healthcare providers, community organizations, and potential referral sources. The Community Liaison promotes the services and benefits of our SNF to the community and facilitates the admission process for prospective residents.

Key Responsibilities

  • Outreach: Conduct presentations, educational sessions, and facility tours for healthcare professionals, community groups, and potential residents and their families.
  • Admission Process Facilitation: Serve as a point of contact for prospective residents and their families. Provide information about our services, admissions criteria, and payment options. Collaborate with the admissions team to ensure a smooth transition for new residents.
  • Community Education: Conduct educational sessions and workshops for healthcare professionals, community organizations, and the public. Increase awareness and understanding of our capabilities, programs, and specialized services.
  • Collaborative Partnerships: Collaborate with local organizations, senior centers, and community groups to foster partnerships and enhance our involvement in community events, health fairs, and initiatives.
  • Compliance: Adhere to legal and regulatory requirements related to long-term care and healthcare privacy. Ensure all activities are in line with ethical standards and organizational policies.

Preferred Qualifications and Skills

  • Bachelor's degree in marketing, healthcare administration, business, or a related field is preferred. RN is highly preferred.
  • Proven experience in healthcare marketing, sales, or community outreach, preferably in a Skilled Nursing Facility or healthcare setting.
  • Strong knowledge of the local healthcare market and community resources.
  • Excellent interpersonal and communication skills, both verbal and written.
  • Ability to build and maintain professional relationships with diverse stakeholders.
  • Effective presentation and public speaking abilities.
  • Strong organizational and time management skills with the ability to multitask and prioritize effectively.
  • Proficient in using computer software and applications for data management, reporting, and presentations.
  • Knowledge of healthcare regulations and compliance standards.
  • Results-oriented with a track record of achieving marketing and referral goals.
  • Ability to work independently and as part of a team.
  • Willingness to travel locally and work flexible hours, including evenings and weekends, as needed.

Working Conditions

  • Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses stations, resident rooms, etc.).
  • Moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
  • Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
  • Communicate with the medical staff, nursing personnel, and other department directors.
  • Works beyond normal working hours and on weekends and holidays when necessary.
  • Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
  • Is involved in community/civic health matters/projects as appropriate.
  • Attends and participates in continuing educational programs.
  • Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS, COVID-19, and Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals.
  • Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents needs.

Education

Must possess and maintain any and all necessary active and unencumbered certifications, degrees, or licenses in the necessary state, to the extent the position requires such certifications, degrees, or licenses.

  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must be a supportive team member, contribute to and be an example of teamwork and team concept.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must be able to relate information concerning a residents condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.

Physical and Sensory Requirements (with or without reasonable accommodation)

  • Must be able to move intermittently throughout the workday.
  • Must be able to read, speak, and understandably write the English language.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies. Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination if the position requires.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
  • Must be able to push, pull, move, and/or lift a minimum of 50 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergencies.
