Intake Coordinator
LOCATION
Remote (work from home in Pennsylvania or Florida)
QUALIFICATIONS
Intake experience required (preferred in home health), current LPN licensure in any state, excellent communication and organizational skills, ability to handle multiple tasks, and proficiency in data entry and reporting.
RESPONSIBILITIES
Receive and respond to inquiries, process referrals, complete intake processes in EMR system, maintain compliance, communicate with physicians and sales staff, run intake reports, and perform other assigned duties.

Job Duties
- Receives and responds to incoming calls and inquiries into agency services
- Efficiently process incoming referrals for all branches
- The Home Health Intake Coordinator completes the full intake process, including but not limited to; inputting the patient's information in to the EMR system( AXXESS), confirming the following Physician, and completing and verifying the F2F
- Understands the regulations and requirements for admission to Home Health
- Communicates with the following physician and patient when needed.
- Coordinates and confirms a valid Face to Face visit nad an order for each referral
- Communicate with Sales and Marketing staff about accepted and declined referrals
- Promote compliance with incoming documentation
- Runs and maintains intake reports and maintains referral tracking sheet
- Demonstrate a desire to set and meet objectives and to find increasingly efficient ways to perform tasks
- Communicates necessary information to the supervisor and management team to ensure coordination of services and activities
- Performs other duties as assigned
Requirements:
- Intake experience is required, home health preferred
- Current LPN licensure in any State
- Ability to handle multiple tasks simultaneously and meet deadlines
- Personal accountability: Self-motivated; organized; maintains confidentiality; complies with all policies and procedures
- Excellent written and verbal communication skills
- Excellent organizational and time management skills
- Proficient with computer skills, data entry and reporting
- Customer service focused
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
Expected hours: 40 per week
