Seeking an Intake Coordinator - Case Manager to join our team! We will work with multiple settings such as clinics, skilled nursing facilities, home health and more. It's definitely an exciting time to be joining our group. We're growing, and with that growth, we're looking to add an experienced Intake Coordinator - Case Manager to our amazing team.

Job Responsibilities

  • Responsible for managing all aspects of the patient intake process including establishing and maintaining positive relationships with patients and referral sources, responding to patients requests and concerns, and maintaining relationships with marketing and clinical staff.
  • Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
  • Sending for insurance verification and utilizes the authorization process
  • Establishes and maintains positive working relationships with current and potential referral sources
  • Gathers, collates, and reports referral statistics including key customer referral trends

Qualifications:


  • High school diploma, and some college preferred.


  • Experience an Intake Coordinator

  • Strong communication and interpersonal skills.

  • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
