The Patient Liaison is responsible for marketing, educating patients and families, managing communication among teams, and ensuring compliance in hospice services to enhance organization reputation and outreach.

JOB DESCRIPTION SUMMARY
Responsible for managing all aspects of organization marketing including establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, explaining and educating on the Medicare Hospice Benefit, obtaining clinical records for referrals, and communication with various members of the team to facilitate the intake process.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Employs effective communication techniques with families, facilities, and community partners to educate on the Medicare Hospice Benefit.
2. Assures that staff understand and adheres to all laws, statutes and regulations regarding anti-kickback, lawful marketing activities (gifts and services), patient solicitation, and accurate marketing materials.
3. Ensures that patients and families are adequately and appropriately educated on their rights and responsibilities on hospice, and that financial risk and responsibility is explained prior to enrollment on hospice.
4. Assists the Performance Improvement team in evaluating existing processes and revamping operations to ensure compliance and regulatory practices are followed.
5. Continuously conducts market assessments and develops a comprehensive marketing plan designed to meet budgetary volume projections.
6. Employs marketing and promotional initiatives to achieve budgetary volume projections.
7. Establishes and maintains positive working relationships with current and potential referral and payer sources.
8. Works collaboratively with internal team members to ensure that the intake and admission proves is functioning seamlessly.
9. Performs in-services with existing community partners as requested.
10. Builds and monitors community, customer, payer and patient perceptions of Calstro Hospice Inc. as a high quality provider of services.
11. Provides leadership in strategic planning including identifying opportunities for additional or improved services to address customer needs.
12. Maintains comprehensive working knowledge of Calstro Hospice Inc. markets including government agencies, major payer groups, key referral sources, and competitor’s market positioning.
13. Maintains comprehensive working knowledge in the field of marketing and shares information with appropriate organization personnel.
14. Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by Calstro Hospice Inc. .
15. Assists in finding placement for patients based on locale and facilitates communication between community partners to ensure safe discharge.

POSITION QUALIFICATIONS
1. Bachelor's degree in Marketing, Business Administration, or related field, Masters degree preferred.
2. Ability to market aggressively and deal tactfully with customers and the community.
3. Knowledge of Medicare Hospice Benefit a plus.
4. Demonstrates good communications skills, negotiation skills, and public relations skills.
5. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
